Establishing Care
For all new patients seeking weight management services at Unique Physique, an initial consultation with one of our licensed clinicians is required. As a medical practice, not a retail pharmacy, our clinicians conduct comprehensive evaluations to determine the appropriateness of prescription medications to support each patient’s weight loss journey. The final decision regarding prescription is based on this professional assessment.
Transferring Care
Patients transferring their weight management care to Unique Physique must provide verifiable documentation of their current prescription. This includes a valid prescription, a copy of the prescription label, or a letter from their previous prescribing provider, clearly stating the patient’s name, date of birth, current medication dosage, and the date of their last order. The decision to prescribe medication remains at the clinician’s discretion following a thorough evaluation during the consultation. Please note that Unique Physique does not guarantee that all patients will meet the clinical criteria for weight loss medications. Furthermore, the prescribing clinician will determine the most suitable medication, considering both brand name and compounded options, to address the individual needs of each patient.
Maintaining Your Relationship with Unique Physique
Billing
- Auto renewal: All patients will be enrolled in auto renew so that their weight loss medications will be properly ordered on week 3 of the first prescription. Occasionally, there are 5-week months. All medications are only supplied in 4-week increments. If a 5-week month occurs, it is the patient’s responsibility to submit a bill date change form under patient accounts. Effective June 15, 2025; any patient who requires a manual order submission (phone, text, etc.) will be assessed a $25 convenience fee. We highly encourage all patients to keep their auto renewal updated and active as a convenient way to receive orders timely, avoid delays and help maintain costs and efficiency.
- Declined Charges: If a charge is declined, you will be notified via text message after all paid orders are processed.
- Refunds/Exchanges: Unique Physique will not refund or exchange medications that were correctly ordered. It is the patient’s responsibility to cancel their account as noted in section “Cancelation.” No pharmacy orders can be canceled or retracted after 48 hours of submission to the pharmacy. If a patient authorizes an order with a stated shipping timeframe of 7–10 business days and subsequently requests cancellation after submission to the pharmacy (for any reason), a fee of up to 25% of the medication cost may be withheld from any applicable refund, provided the pharmacy is able to cancel or intercept the order prior to shipment. This fee is intended to cover administrative, restocking, and shipping-related expenses, including but not limited to processing time, preprinted shipping labels, and packaging materials.
Allergic Reactions / Injection Site Reactions Policy: All medications, whether oral or injectable, carry an inherent risk of allergic reactions or localized injection site reactions. These responses are well-documented in clinical literature and are not unique to medications administered at Unique Physique, LLC.
Localized reactions—such as redness, swelling, itching, or tenderness at the injection site—are among the most common adverse effects associated with injectable medications, occurring in approximately 10% to 20% of patients depending on the drug and route of administration [1]. More serious allergic reactions, including anaphylaxis, are rare but possible, with an estimated incidence of 1 to 3 cases per 10,000 exposures for most medications [2]. These reactions can develop even in individuals with no prior history of allergies and may occur unpredictably. As noted by Kelso et al. in the Journal of Allergy and Clinical Immunology, anaphylactic reactions can occur on first exposure and do not always correlate with previous tolerance to the same medication [3].
Unique Physique, LLC thoroughly reviews patient medical histories and performs individualized assessments prior to initiating therapy. However, due to the unpredictable nature of allergic responses and injection site reactions, we cannot guarantee how any individual may respond to a given medication.As such, Unique Physique, LLC does not offer refunds or replace medicines in the event of an allergic or injection site reaction. We strongly encourage patients to report any concerning or unusual symptoms promptly so that appropriate medical evaluation and care can be provided. A visit with a clinician may be necessary and a new medication may need to be prescribed, which is at the clinicians discretion.

- Cancellation: You are not in a contract. To cancel your account at any time, visit “patient accounts” and complete a cancellation request. Cancellations must be submitted before 5 pm CST the day before billing. Effective June 15, 2025; Patients who cancel and restart services between days 1- 59 of cancelation will be assessed a $15.00 reengagement fee. The timeframe is calculated from the submission date of the cancellation form- not medication last usage date. After 60 days from your cancellation date (not medication completion date) you must schedule a new consultation.
- Restarting services: Effective June 15, 2025. Due to the nature of weight loss medications, if a patient cancels services they have 60 days to restart without needing a new consultation and reevaluation. Patients returning between 0 and 60 days will have their chart reviewed by a clinician and their dosage could be lowered if a valid prescription for the same medication cannot be provided for review . The prescription must be written for that patient with clear patient identification, during that 60-day period. Patients who return for services after 61 days of cancellation will be required to schedule a new patient consultation at their expense. The timeframe is calculated from the submission date of the cancellation, not patient self-report of when they finished their last vial of medication that was provided by Unique Physique.

Medication Adjustments & Ordering
Dosage adjustments: All medications are dosed differently and have a different threshold for when they need to be increased or not. Patients should review the Unique Physique protocols provided to them in their welcome letter at consultation. Requests for dosage adjustments outside of our standard protocol must be requested using this link in patient accounts. Dosage adjustments must be submitted before 5 pm CST prior to billing date and are at the discretion of the clinician prescribing. Dosage adjustments are not guaranteed, and Unique Physique does not allow self-directed care by patients. Patients who are on maintenance or are taking medications in a different frequency than weekly will not be eligible for dosage increases. Patients who place infrequent orders will not be eligible for dosage increases until they return to weekly dosing and the clinician approves a subsequent increase.
Patient Specific Compounding vs. Traditional Pharmacy Prescribing
Our clinician will review all signs and symptoms and determine if a patient would benefit from brand name weight loss medications or a compounded version to prevent or alleviate certain signs and symptoms occurring with brand name medications. If a brand name medication is recommended, the patient will be assessed a $25 fee monthly to submit the prescription to the patient’s pharmacy of choice. It is the patient’s responsibility to call monthly for the refill. No prior authorizations will be performed. In the event compounded medication is needed or if a change to that compound is required by the patient – ie. symptoms resolve, symptoms are not being managed, patient has an allergy or reaction to compounded ingredients; a new consultation will be required with a clinician.
Ordering Early / 28-Day Supplies
All pharmacies reserve the right to NOT refill a prescription until the appropriate time frame based on the number of days supply originally ordered.
Misuse/Misappropriation of Medications
Effective 5/1/25, all prescriptions will be a 4-week supply with a 28 day expiration from vial puncture – unless noted otherwise in the pharmacies literature.
Maintenance
Patients who are on maintenance, or taking medications in a different frequency than weekly will not be eligible for dosage increases. If a patient chooses to take their medication every other week which extends the medication beyond the 28-day puncture rule that medications will not be replaced secondary to expiration.
It is the patient’s responsibility to read the label on their medication in its entirety. These are your dosing instructions that are printed on the label. Do not use instructions on our example photos, instructions from other patients, or instructions from other clinics or pharmacies. Each prescription is written specifically for the patient intended. If you feel that there is an error in the written instructions, please contact the office BEFORE injecting. Any mistakes in injections resulting from failure to read the instructions correctly or call the office to clarify, will result in an additional medication charge if more medication must be dispensed/ordered. Please remember this is PRESCRIPTION MEDICINE and should be taken as instructed by the prescribing clinician. Patients are not allowed to self-medicate or raise their dose of medication without approval from a clinician. Self-directed care, misappropriation or other activities that violate our prescribing policies will result in immediate patient termination.
