Request Dosage Increases

Please submit your request before your billing date. Requests received on your billing date will take effect on the next billing cycle.

Change Your Shipping Address

Ensure you submit address changes before your billing date. Do not submit changes on the billing date.

Change Your Payment Date

Please note we only bill on the 1st, 7th, 14th, 21st, 28th. Payment date changes must be made before 5pm on the day before you are billed.

Request documentation for HSA/FSA

Please use this form to request documentation for your HSA/FSA. Unique Physique DOES NOT provide letters of medical necessity. Please allow 3-5 business days from time of submission for the document to be emailed to you.
